This record series consists of a three-part annual report (Annual Financial, Program Section, Vocational Education) completed by each individual school district's business administrator for the previous fiscal year. It is submitted by the school districts to the State Office of Education, School Finance and Business Section at year's end to receive allotted funds due. Information includes name of school district, name of person that prepared report, date the report was prepared, signature of the superintendent, name and telephone number of auditing firm, date received by the business office, date sent to data processing, date returned from data processing, annual financial balance sheet, progress section balance sheet, and vocational education balance sheet. Utah Code Annotated 53A-3-403 (1989) governs the date that each report shall be forwarded to the State Superintendent's office. Information collected from this report is compiled in part in the Annual Report of the State Superintendent of Financial Instruction (83278) which is a permanent document, however, this record series contains significant detail which the Superintendent's annual report summarizes in broader, less specific form. In working with the local school districts it has been determined that the Office of Education will be recognized as holding the official record copy.
