Milford (Utah) Cemetery records, 1913-2002.

This series contains books of forms recording burial information for individuals interred in the Milford cemetery. The information recorded on the forms may include name of deceased, sex, race, date of birth, where born, residence, occupation, marital status; parents names and dates of birth; number of burial certificate; place of death; date of death; cause of death; name of certifying physician; date of funeral and undertaker. It also includes the plot, lot, and grave location for the individual along with ownership information on the burial plot. In many cases there are Burial-Transit Permits attached to the forms and occasionally there are other documents attached, such as death certificates, obituaries, or disinterment permits. One book also contains a list that appears to be a list of grave plot purchases, listing the name of the lot, the purchaser, the amount paid, etc.
