Scope and Content
This series contains an index book created for the purpose of locating court actions recorded in the probate record books. The index is by the name of parties involved in probate proceedings, along with the books and pages where actions in those proceedings are recorded. This index covers Probate Record Books 1, 2, and part of Book 3. In the index the page numbers for Books 2 and 3 are clearly identified. Most unspecified page numbers refer to Book 1, but a few of the earliest entries seem to refer to an unknown book.

Custody History
The court transferred the book in this series to the State Archives 2007. It was sent to the regional repository at Southern Utah University Special Collections in November 2009. The University Special Collections returned the book to the State Archives in October 2025.

This series was archivally processed by Jim Kichas and Emily Gurr in June 2008 as part of a NHPRC grant project designed to preserve the historic records of Utah's Fifth District Courts.
