Summary of Records
Scope and Content
This series contains reports of student grades and attendance for each class in the district. The forms, identified as Form 110, were completed by teachers at the end of the school year. The forms include such information as the teacher name, textbooks used, names of students in the class, number of days each student attended, and grades given. The reports include those submitted from elementary schools, high schools, and adult education classes.

Alphabetical by school name, thereunder chronological by school year.
Access Restrictions
This series is primarily classified as Exempt: Academic records are restricted under the federal Family Educational Rights and Privacy Act (FERPA) as long as the student is alive.. The secondary classification is Private: Records of minors under the age of 21 are considered private for 100 years, when the subject is presumed to be deceased. (Utah Code 63G-2-310).
