Schedule Description

This series contains the local copy of affidavits for amending the information on birth certificates issued in Weber County.

Scope and Content

This series contains the local copy of affidavits for amending the information on birth certificates issued in Ogden. The affidavits appear on forms created by the State Department of Health and were used to correct or amend information on the original birth certificate. The affidavits were most commonly used to add the name of the child, when the child's name had not been provided at birth. They were also used to correct parents' names and even birth dates. In most cases these affidavits were filed decades after the creation of the original birth certificate.
The forms include space to note such information as the original birth certificate number, date of birth, original information, corrected or amended information, and the reason for the change. The form also provides space for the signature of two witnesses attesting to the veracity of the information and the signature of the local registrar. The original forms were sent to the State Department of Health and the copies in this series were kept for local use.
The series also contains a single amendment to a death certificate for Ethel Hill, who died in August 1920.
