Schedule Description

These records support the agency's function to document and certify the facts of deaths for the legal purposes of the state's citizens. Records contain orders arising from a court action judicially establishing the fact, time, and place of a death which is not registered, or for which a certified copy of the death certificate cannot be obtained. Records include information about the deceased, the cause of death, disposition of remains, and related information; includes an index.

Scope and Content

This is an order arising from a court action judicially establishing the fact, time, and place of a death which is not registered or for which a certified copy of the death certificate is not obtainable. This is authorized by UCA 26-2-15. The information includes the deceased's name, sex, date of death, race, age, birth date, address, birth place, citizenship, marital status, spouse's name, social security number, parents' name, and residence; the name and address of the informant; the cause of death; the name of the funeral director and cemetery or crematory; and the signature of the registrar.
