This series contains report forms submitted to the county clerk in accordance with a law passed in 1898, requiring that "all physicians and professional midwives must keep a register of each birth at which times they assist professionally" and that a certified copy of their registers must be filed quarterly with the county clerk (1898 Utah Revised Statutes, Title 57).
Tooele County (Utah). County Clerk Birth reports
This series contains report forms submitted to the county clerk in accordance with a law passed in 1898, requiring that "all physicians and professional midwives must keep a register of each birth at which times they assist professionally" and that a certified copy of their registers must be filed quarterly with the county clerk (1898 Utah Revised Statutes, Title 57). The county clerk used the submitted forms to compile a listing of all births in Tooele County in the official "Register of Births" (Series 84240). The forms include such information as the location of births, child's sex, color, race, date and place of birth, parents' names and residence, name of informant, date of report, signature and address of informant, and date filed with the clerk. These forms were no longer used after 1905, when the system of reporting births was revised to place responsibility for registering births with the State Health Department.
