Tooele County (Utah). County Clerk Death reports
This series contains report forms submitted by physicians or persons in attendance at deaths in accordance with state statute. The submitted forms were used by the clerk to maintain a register of deaths in Tooele County. Each form contains space for the recording of six deaths. The reports include name of person making report, number of death, name of deceased, date of death, place of death, length of residence, sex, color, race, age, occupation, cause of death, marital status, birth place, last place of residence, name of informant, and date filed with clerk.
